If you have questions about eligibility or documentation guidance, please click the link below to review our website. The purpose of completing this application is to help your Disability Specialist get to know you better and to understand how your disability impacts you. Providing more information in advance of your appointment will help expedite the registration process.

If you need testing accommodations for the Math Placement Test please complete the application and email to initiate the process of getting Math Placement accommodations. If found eligible, you will be provided a letter to share with the Math department.
