The purpose of completing this application is to help your Disability Specialist get to know you better and to understand how your disability impacts you. Providing more information in advance of your appointment will help expedite the registration process.

If you need testing accommodations for the Math Placement Test please complete the application and email adsfrontdesk@umd.edu to initiate the process of getting Math Placement accommodations. If found eligible, you will be provided a letter to share with the Math department.
In order to access this application you must be a current UMD student or have been admitted to UMD and confirmed your offer of admission. Please have your UMD Directory ID and Password available to access the form. Incoming students will be able to acquire their Directory ID within 48 hours of their admission decision. A Directory ID typically consists of letters; it is different from your UID which is 9 digits. You will also need to provide your UID in the form.
This Application should only be completed ONCE. If you are already registered with ADS and have a new disability or your accommodation needs have changed, please contact your assigned Disability Specialist. If you do not know who your assigned Specialist is, email the ADS Front Desk for assistance (adsfrontdesk@umd.edu).
Please Note: By clicking Register and completing this application, you have only completed the first step in the registration process. You will not be registered with the ADS office until you complete the following 4 steps:
Steps to Register with ADS to Receive Accommodations
1. Complete online Registration Application
2. Submit documentation
3. Call the ADS Office to schedule a registration appointment (you do not need to make an appointment if you ONLY need housing accommodations)
4. Meet with a Disability Specialist to determine eligibility and establish appropriate accommodations
You, the student, must complete this application in one sitting; you will not be able to save and return to it. Please allow 15-20 minutes to complete this application.
You should have available:
· Directory ID and Password as well as your University ID Number (UID)
· Information about your disability including types of accommodations you have used
· Documentation of your disability to upload
Please Note: Registration Applications will only be kept for 1 year. If you do not schedule an appointment with ADS within 1 year of completing this application, it will be removed from our system and you will need to re-complete it in order to register with our office.
